Dopamine agonists mimic dopamine, a neurotransmitter in the brain that plays a crucial role in movement, pleasure, and reward. By stimulating dopamine receptors, these drugs help alleviate symptoms like tremors and muscle stiffness in Parkinson’s patients and reduce the involuntary leg movements associated with restless leg syndrome. However, this artificial stimulation of dopamine pathways can overactivate the brain’s reward system, leading to impulse control disorders (ICDs). Studies suggest that 13% to 24% of Parkinson’s patients on dopamine agonists develop ICDs, which can manifest as excessive gambling, compulsive shopping, or hypersexual behavior.

While these side effects are listed in the drug’s prescribing information, they are often overlooked or underestimated by healthcare providers. Many patients report feeling a disconnect between their actions and their sense of self, describing an internal struggle to resist overwhelming urges. For some, the consequences of these behaviors are life-altering, leading to financial ruin, broken relationships, and even criminal charges.


Case Studies: Patients Struggling with Medication-Induced Behaviors

The BBC reviewed several harrowing cases of patients who experienced devastating side effects after being prescribed dopamine agonists. One striking example involved a 63-year-old man who was treated with Ropinirole, a dopamine agonist manufactured by GSK. The man, who had no prior history of sexual misconduct, sexually assaulted a 7-year-old girl while on the medication. His libido had increased significantly after starting the drug, and the behavior ceased only when his dose was reduced.

For women, the effects of these drugs have been equally transformative. Claire, a woman prescribed Ropinirole for restless leg syndrome, began experiencing uncontrollable sexual urges about a year into her treatment. She recounted leaving her house in the early hours of the morning to flash strangers, despite being in a committed relationship. “There’s an element in your head that knows what you’re doing is wrong, but it affects you to the point that you don’t know you’re doing it,” she explained. It took years for Claire to link her behavior to the medication, and the urges disappeared immediately when she stopped taking it. However, the shame and guilt she feels remain.

Sarah, another woman who took a different dopamine agonist, described how her previously low sex drive skyrocketed into an addiction. A company report from that year, reviewed by the BBC, documented two cases of men prescribed Ropinirole who exhibited “problematic” sexual behavior. Despite this knowledge, patients and their doctors claim that the risks were not adequately communicated. Many patients report feeling blindsided by the side effects, which they were not warned about before starting the medication.

In one notable case, a class-action lawsuit was filed against GSK in 2011 by four Parkinson’s patients who claimed that Ropinirole had led to gambling debts and broken relationships. The plaintiffs argued that GSK had failed to include warnings about these behaviors in the drug’s literature until 2007, despite studies linking the medication to such risks as early as 2000. The case was eventually settled out of court, but GSK denied liability.


GSK’s Response and the Ongoing Debate Over Responsibility

GSK has defended its handling of the situation, stating that Ropinirole has been prescribed for over 17 million treatments worldwide and has undergone extensive clinical trials.
